A $6,000 Hawaii vacation for two is a real financial commitment. Then you see the travel insurance add-on at checkout — another $300 to $500 — and the mental math starts: is this worth it, or am I just paying for peace of mind I don’t need?

The honest answer depends on what you’re doing in Hawaiʻi, how you booked it, and what your credit card already covers. Most people either buy too much insurance or skip it entirely. Both are mistakes. Here’s how to figure out where you actually land.

A standard comprehensive travel insurance policy bundles four things into one premium:

Trip cancellation and interruption. Reimburses prepaid, nonrefundable costs if you have to cancel or cut your trip short for a covered reason — illness, injury, death in the family, jury duty, job loss, or severe weather. Typically covers 100% of your prepaid trip cost.

Emergency medical expenses. Pays for emergency treatment — ER visits, hospital stays, doctor consultations, prescription drugs — while you’re traveling. Limits usually range from $50,000 to $250,000 depending on the plan.

Emergency medical evacuation. Covers the cost of getting you from a remote location to a hospital, or from a local hospital to a better-equipped facility. This is the coverage that matters most in Hawaiʻi, and most people don’t realize how expensive it is without insurance.

Baggage loss and travel delay. Reimburses you for lost luggage, delayed bags, or trip delays that strand you overnight. Limits are typically modest — $500 to $1,500 for bags, $150 to $300 per day for delays.

Most providers also offer a “cancel for any reason” (CFAR) upgrade, which lets you cancel for reasons not on the standard covered list — cold feet, schedule change, just don’t feel like going. It adds roughly 40–50% to the premium and typically reimburses 50–75% of your costs. Hawaiʻi is not Cancún. The things that go wrong here are expensive in ways that catch mainland travelers off guard.

Helicopter tour accidents. Tour operators require you to sign a liability waiver before boarding. The rescue itself is currently free (Hawaiʻi fire departments and DLNR don’t bill, though state lawmakers have repeatedly floated bills to change that). But once you’re moved to a hospital, the bill arrives. Ocean injuries. Drowning is the leading cause of injury-related death for visitors in Hawaiʻi. Roughly 69% of all ocean drowning victims in Hawaiʻi are visitors, with the state averaging about 45 visitor ocean drownings per year per the 2025 Hawaiʻi Water Safety Plan. Non-fatal incidents — near-drownings, spinal injuries from shorebreak, coral lacerations requiring ER treatment, jellyfish stings with allergic reactions — are far more common. An ER visit without insurance averages $2,200 to $3,000 nationally, and Hawaiʻi’s cost of living pushes that higher.

Volcanic activity. Kīlauea erupts regularly. The 2018 eruption forced mass evacuations in lower Puna on the Big Island and closed sections of Hawaiʻi Volcanoes National Park for months. Vog (volcanic smog) can trigger flight delays and respiratory issues. Standard trip cancellation policies cover disruptions caused by volcanic activity if they’re declared by local authorities.

Hurricane season. June through November. The Central Pacific Hurricane Center tracks systems that approach Hawaiʻi. Hurricane Lane in 2018 is the clearest recent example of a storm that caused widespread flight cancellations, hotel operational disruptions, and visitor displacement across the islands — but near-misses and tropical storm brushes happen every few seasons. Trip cancellation coverage kicks in when a named storm forces airline schedule changes or mandatory evacuations.

Premium credit cards advertise travel protection, and some of it is genuinely useful. But card benefits are not travel insurance, and the gaps matter most in Hawaiʻi.

The Chase Sapphire Reserve — probably the most common premium travel card — offers trip cancellation up to $10,000 per person, trip delay reimbursement up to $500, and Emergency Evacuation and Transportation coverage up to $100,000. That’s real coverage for the plane ride or ambulance ride itself. Most premium travel cards follow this pattern: they protect the trip investment and the logistics of getting you to a hospital, but not the cost of the care itself.

If your domestic health insurance covers out-of-state emergencies cleanly and you’re doing a low-risk Hawaiʻi vacation (resort, beach, restaurant, repeat), the card’s cancellation and evacuation coverage might be enough. Younger travelers pay closer to 4%; travelers in their 70s can pay 10% or more for the same trip.

Here’s the math that makes the decision easier:

Scenario 1 — Trip cancellation. You book $3,000 in nonrefundable flights and $2,500 in nonrefundable hotel nights. Your partner gets sick two days before departure. Without insurance: $5,500 gone. With insurance: full reimbursement minus the premium you already paid.

Scenario 2 — Medical evacuation. You slip on a wet trail on the Big Island and break your ankle at a remote trailhead. Helicopter life-flight to Hilo Medical Center: easily $20,000 or more. With insurance (or a card like Chase Sapphire Reserve with a 5+ day trip qualifier met): covered under the evacuation benefit. Without either: the bill lands on you.

Scenario 3 — Hurricane disruption. A tropical storm shifts course toward Maui. Airlines cancel your flights. Your resort shuts down. Without insurance: you eat the hotel nights and rebook at inflated last-minute prices. With insurance: trip interruption covers unused hotel nights and rebooking costs.

The pattern is simple: if nothing goes wrong, you’re out $300. If something goes wrong without insurance, you’re out thousands to tens of thousands. Travel insurance is a bet on low-probability, high-cost events — and Hawaiʻi has more of those events than most domestic destinations.

Whichever of the three you start with, the policy details are what actually matter. Not all are equal. Check these specific things for a Hawaiʻi trip:

Insurance isn’t a universal requirement. You can reasonably skip standalone travel insurance for a Hawaiʻi trip if all of the following are true:

Your flights and hotel are fully refundable or rebooked at no cost. Your credit card provides trip cancellation up to your trip value. You have good domestic health insurance that covers out-of-state emergencies (check this — some HMOs do not). You’re not planning any high-risk activities: no helicopter tours, no remote hiking, no open-water snorkeling or surfing.

That describes a pretty specific trip — a short, resort-based vacation booked on refundable rates with a premium credit card. For most Hawaiʻi trips, at least one of those conditions doesn’t hold, and that’s when standalone coverage earns its premium.

One more thing worth knowing: Hawaiʻi’s Prepaid Health Care Act mandates employer-provided health coverage for Hawaiʻi employees, but that’s a workplace benefit — it doesn’t extend to visitors. The islands are remote, the terrain is rough, and the activities that draw visitors (hiking, helicopter tours, ocean sports) are exactly the activities where things occasionally go sideways. A roughly $200–$300 comprehensive plan with solid medical and evacuation limits is the single best financial hedge you can make on a Hawaiʻi trip.
